Request a quilt for a Cancer Patient or Facility. This short procedure helps us ensure accurate information. You only need to register one time, and you can visit your account at any time by logging in through Log Into Your Account on the Request A Quilt page. Once you have established an account, please do not email changes and updates to us. Use your account to notify us of any changes to your request, such as a change of address.

Information that we collect include:

• Your name, email, address and phone number in case we have a question.
• The Recipient's name, address (can be your own), phone number(can be your own) and size of quilt (child, teen, adult). If you are requesting a quilt for yourself, please fill in both the requestor and recipient information.
• Hospital or care facility name (if any) and address if you want the quilt to go directly to the hospital

Be sure to make a note of your username and password.
